Dear Mr. Gurijala,
Your company has been providing me health insurance for three years now. Based on my application, the insurance policy is set to expire on April 16. I have been satisfied with your service and have had no problems with insurance claims but I regret to inform you that I am cancelling my health insurance with your company. This is because I will be relocating to another country soon. The cancellation is effective one month after receipt of my letter which is as stated in the insurance policy I signed.
I am aware that I will be receiving a certain sum when I cancel my health insurance with you. I will be paying my last payment this month and I hope to receive the closing amount.
Please send me a letter to acknowledge the cancellation.
