Having received your letter dated June 8, I was surprised to note that you state that my medical insurance will not pay for the operation I have had on my ankle. I would point out that the surgeon’s assistant, Ms Hunt, phoned for authorization from yourselves on April 9. Ms Hunt was provided with the authorization reference 4452A. As such I would ask that you honor this authorization and pay the cost of 80% of the operation as per the terms of my insurance.
 

Having already communicated with the State Insurance Commissioner, I have it confirmed that this authorization is also a guarantee that the bill will be paid by yourselves. Having always been satisfied with the service provided by your company, I would trust this mistake will be resolved as soon as possible.
