Appraisal letters are given to notify employees about their good work along with the salary increment given to them.  With these kind of letters the other employee might also want to work hard and get the same some day.

I am pleased to inform you that following your recent employee evaluation, the committee has decided to increase your salary in line with your performance.

This will be effective from _____________ (date), your monthly salary will rise to __________ (amount) which reflects a ______ percent increase.
