Dear Mr. Adiga,
I write to you to ask for a holiday on February 15. I had a medical check-up last week and my physician found a small lump at my neck. He has recommended that I have laboratory tests including a biopsy on the said date to find out what the lump is and possibly rule out the possibility of a cancer.
I will make myself available to undergo whatever procedure needed so that I may be allowed to have a holiday for my laboratory tests. I have already begun the paper work assigned to me during the last staff meeting and I am sure that I will be able to finish all my work before I take a holiday on February 15. Please inform me if I must accomplish additional tasks before the said date.
