Dear Mather Smith,

I am writing to inform you of an error which I have spotted in my insurance policy. The policy which I received last week states that I have only one beneficiary. I don’t know what could have happened, but I did, in fact, submit two names and two separate sets of documents to cover two beneficiaries.

The primary beneficiary, whose name already appears in the policy, is my daughter, while the second beneficiary is my youngest son. Please go over the attached copy of his records.

Should you have any questions or concerns, please feel free to contact me at 434-3434. Thank you for your assistance.

Sincerely,

Moses
