Dear Mary Smith,

On Friday, January 6th, I had a routine teeth cleaning by my dentist of record, Dr. Sanbourne. When I left the office, the receptionist said they were filing the claim as usual.

Today, however, I received a call from Dr. Sanbourne’s office stating that my coverage had been denied and I owed them for the whole cost of the visit. Please check your records and tell me what the problem is. I know my payments are current and I received no services that were out of the ordinary. I have never paid anything for a routine cleaning and I am curious to why I would have to now.

I feel it is likely that somehow the claim was simply misfiled. I hope you can find the answer and remedy it quickly.
