I am writing you this letter to get medical authorization from Tata Hills Pvt. Ltd as I am working here as Marketing Manager- Marketing Department since August 1st, 2007. At the time of joining, I was assured by the HR department of getting medical authorization from company underneath I can use certain medical benefits. You can find the enclosed copy of my joining letter for your ready reference.
As you are aware, I have completed my tenure of three years with this company and according to legal clause, I am eligible for medical authorization. Kindly let me know the procedure to complete this authorization and if any document is needed for the same. I have already submitted my medical records and latest salary slip to HR department as I was informed yesterday by HR Executive.
